Drugs and other forms of medicine have been referenced frequently throughout the Homestar Runner universe, in both medicinal and recreational contexts.

Medicinal Drugs
- Email autobiography — In "Words I Probably Said", Strong Bad explains that "somebody must have slipped [Reg] some drugs or something before the game".
- Email funny — Strong Sad asks Strong Bad, "Did you take some of my pills again?"
- Peasant's Quest — One of the items you give The Three Keepers of Trogdor is a bottle of pills.
- Email time capsule — One of the Easter eggs features Jobar's Big Ol' Headache Medicine.
- Stinkoman 20X6 — The main item for restoring health is a large pill.
- Email candy product — Strong Bad's junkmail filter, Edgar's Baby's Daddy, has one email with the subject "Re: PharamBlacy news", parodying spammers who advertise prescription drugs.
- Email alternate universe — Homestar Runner tries to make a real fruit smoothie using "Total Load", presumably vitamin supplement pills.
- No Hands On Deck! — Strong Sad states that he "needs to get his prescription changed".
- Email retirement — Strong Bad's "Cloud Shape Spotting Log" in an Easter egg lists "diet pills".

- Scroll Button Songs — Coach Z raps, "And if you're tryin' to fade me then ya must smoke crack!"
- Limozeen Thanksgiving E-Cards — Gary Palaroncini says, "Kids under six, don't drink and drive! And drugs, stay out of school!".
- 4 branches — Homestar's "deadbeat boyfriend" shirt bears a striking similarity to a "Legalize Marijuana" T-shirt.
- mascot — On a similar note, Strongbad explains that he, the Cheat, and Strong Mad were really hungry and tired and sorta confused" when they decided upon the Dumple as the Crazy Go Nuts University mascot. Strong Bad thinks that they "may have meant dumplings" but "ended up putting Dumples on the application". Hunger, lethargy, and confusion (especially in combination with one another) carry classic associations with the use of marijuana.
